Motivation

Herald's #328 retitled docs/cold-read-prompt.md H1 to 'Changelog Cold-Read Prompt' for glance-collision avoidance (their commit 2b07ddf). Reciprocal disambiguation needed on the sibling doc:

docs/internal/cold-read/README.md currently has generic 'Cold-read prompt template' title
A maintainer scanning wouldn't know the changelog cold-read lives at docs/cold-read-prompt.md

Scope

Retitle docs/internal/cold-read/README.md H1 to signal maintainer-scope (e.g., 'Maintainer Cold-Read Framework — README/docs skeptical-outsider audit')
Add one-line pointer to docs/cold-read-prompt.md for the changelog-verifier sibling
Same discipline shape as Herald's #328 rename-at-second-surface

Coordination

Small mechanical, either Herald or QM can fold. Priority/low, size/S. Post-Set-J close is fine.
